Step 1

Print our Account Information Form

• Complete and sign the form
• Fax the completed, signed Account Information Form to: 918-664-4366

As a Consumer Reporting Agency, USIS must follow the guidelines of the Fair Credit Report Act (FCRA), the Driver Privacy Protection Act (DPPA) and other applicable state and federal laws. As a result, USIS members must provide information on their companies, sign a set of agreements and be verified before establishing an ordering account.

Step 2

Once USIS receives your Account Information Form, a USIS Representative will contact you to discuss your needs and send you the appropriate forms. A full set of agreements will be needed based on the states in which you hire. The states have different requirements governing access to the information they provide.

Step 3

After USIS receives your signed and completed forms, your account will be verified using the information provided on the Account Information Form. You will be issued an account number and provided with ordering instructions upon completion of the verification process.
